What to bring.
Bring what you have. If something is missing, we’ll tell you how to get it. In most cases, we can prepare the file the same day once everything is in hand.
Use this as a starting checklist. The exact documents depend on your account, vehicle, and application.
Call (380) 269-7408- Photo ID & company recordsThe signing owner’s ID, legal business name, tax identification records, and Ohio company registration documents, as applicable.
- Ohio base documentationRecords showing the qualifying Ohio business location or residency. Follow BMV 4856 for the applicable evidence.
- Vehicle title, front and backFor each truck, plus its VIN, purchase or lease date, and weight information.
- USDOT & carrier informationYour USDOT number, MC number if applicable, and the details of the carrier responsible for safety.
- Lease & owner authorizationFor leased vehicles, bring the lease and relevant owner authorization. If operating under another carrier, bring that agreement too.
- Form 2290 Schedule 1The current IRS-stamped record, when required. Tell us if the vehicle was recently purchased so we can review which documents apply.
- Mileage, renewal notice & cab cardsFor an existing account, bring the account and fleet numbers, relevant mileage records, renewal notice, and current cab cards.

Establishing Ohio base jurisdiction
Use this form to document your qualifying Ohio business location or residency. Follow the applicable section and include its supporting records.

For authorization from a titled owner to register a leased vehicle. Bring the lease and title as well; the authorization is not a substitute for those records.

Identifies the carrier responsible for the vehicle’s safety. The carrier’s authorized representative must sign; a licensing agent or unrelated preparer cannot sign for them.

Form 2290 generally applies to highway vehicles with a taxable gross weight of 55,000 pounds or more. Bring your current IRS-stamped Schedule 1 when required for registration. The IRS page has the current form and instructions.
