Start here for a new Ohio IRP account. The packet includes the registrant, vehicle, and distance schedules, instructions, and supporting-document requirements.
Open new account packet — official PDFUse this form to document your qualifying Ohio business location or residency. Follow the applicable section and include its supporting records.
Open base jurisdiction form — official PDFFor authorization from a titled owner to register a leased vehicle. Bring the lease and title as well; the authorization is not a substitute for those records.
Open owner authorization — official PDFIdentifies the carrier responsible for the vehicle’s safety. The carrier’s authorized representative must sign; a licensing agent or unrelated preparer cannot sign for them.
Open carrier safety statement — official PDFFor existing Ohio IRP accounts. Used with the relevant schedules for an added vehicle or fleet, a registration transfer, or certain cab card updates.
Open fleet management form — official PDFRecords the details of each truck: VIN, make, year, weight, ownership, and carrier information. Have a clear title copy available when completing it.
Open vehicle schedule — official PDFRecords jurisdiction mileage and requested registration weights. Follow the instructions for your fleet’s actual operating history; do not invent mileage estimates.
Open distance and weight schedule — official PDFRead alongside your fleet’s renewal application. Use the reporting period shown on your renewal paperwork when assembling the distance records.
Open renewal instructions — official PDFFor a lost, stolen, or destroyed apportioned plate. Check the form for the information and declarations required for your situation.
Open replacement plate form — official PDFForm 2290 generally applies to highway vehicles with a taxable gross weight of 55,000 pounds or more. Bring your current IRS-stamped Schedule 1 when required for registration. The IRS page has the current form and instructions.
